Wimbledon Guild Outreach Worker

The Wimbledon Guild’s vision is for every adult in Merton to get the most out of life and experience mental, physical and emotional wellbeing. Through our Outreach service we meet with people, often in their own homes, to understand their needs and aspirations and create individualised programmes of support with them.

As one of our Outreach Workers you will be involved in carrying out assessments with clients to help them define and understand their needs and aspirations.  You will develop individual plans with them to help improve their wellbeing, and take responsibility for helping clients follow through on these. You will regularly review progress with each client and agree next steps, and regularly report on outcomes by collating case records.

We are looking for someone with strong interpersonal skills who can build relationships with clients and partner organisations, who can communicate sensitively and effectively. You will have a positive view of people who face difficult transitions and situations in their lives and a strong desire to empower and enable them. You will be able to travel throughout Merton and be able to use IT including databases.
